Mark your calendar for Thursday, May 13 to receive your COVID-19 vaccine at either of our Community Centers.

At the Madison Cross Roads Community Center, the clinic will be from 10am until 1pm for the Johnson & Johnson vaccine. Appointments will be required. Please call (256)813-3036 to schedule your appointment at the Madison Cross Roads Community Center. Participants in this clinic must complete this form and bring with them to receive the vaccine.

At the Monrovia Community Center, you will have the choice of the one dose Johnson & Johnson vaccine or the two dose Moderna vaccine.  The clinic will be from 9am until 2:00pm and will be drive through or walk in.  No appointment necessary.
