To record a DD214:

• Bring original DD214 (Marked Member 4 in bottom right corner) to Probate Judge’s Office
• There is no charge for the initial recording. You will be provided one complimentary certified copy at the time of recording with original to be returned to you by mail in approximately 7-10 days.

To acquire a copy of a DD214:

• Certified copies can be obtained upon presentation of proper identification
• Please contact us at 256-532-3330 or probate@madisoncountyal.gov to obtain your copy

$2.00 per certified copy
