Citizens will have the choice of the one dose Johnson & Johnson vaccine or the two dose Moderna vaccine.  The clinic will be walk in only.  No appointment necessary.  Must be 18 years old  or older to receive either of these vaccines.

Participants must complete this form and bring with them to receive the vaccine.
