Madison County is considered a large employer under the Affordable Care Act (ACA) and the Form 1095-C provides monthly detailed information regarding employer-provided health insurance coverage or offers of coverage to the employee and any eligible dependents.
The Personnel Department will provide employees with a completed Form 1095-C by March 2, 2017, the IRS deadline. You may complete your 2016 Federal tax return prior to receiving the Form 1095-C. If you had health care coverage for the entire year for yourself and everyone in your tax household, you will simply check the box on your individual income tax return to indicate full year coverage. Keep the Form 1095-C for your tax records only. Do not mail the form to the IRS with your tax return.
