You must call 256-532-3345 for an appointment.
When your appointment has been scheduled, you must bring with you the following items:

• Proof of income for everyone living in household
  • Copy of last year's tax return
  • Social security award letter
  • Monthly social security check
  • Bank statement
  • Savings statement
• Proof of identity
• Medicare card
• Medicaid card
• Social security card
• List of current medications
• Physician's name, address and phone number
